29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section On or about February 6, 2018, the employer failed to ensure that the written Lock Out Tag Out Procedures for their various machinery included the scope, purpose, rules, and techniques required for performing lock out tag out on their machinery. Machinery including but not limited to: the Mazak Integrex 50 CNC Lathe machine.
