29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: (a) Alphas Roofing at 11076 Telluride Ct. Lot 71 Commerce City, CO 80022: On or before January 16, 2018 the exposing and creating employer did not protect employees from injury, in that on January 13, 2018 an employee installing drip edge on a two-story residential home sustained serious injuries after falling approximately 18 feet to the ground. The employee wore a full body harness which was connected to a lifeline via a shock-absorbing lanyard and rope grab. This condition exposed employees to a fall hazard of approximately 18 feet.
