Text For Citation: 01 Item/Group: 001A Hazard:
29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. Extrusion department, portable unknown/unmarked sheet shear: On or about January 2, 2018, operators were assigned to perform setup tasks (servicing) at the point-of-operation and associated danger zones when the machine-specific hazardous energy control (lockout tagout) procedure was not developed, documented, and utilized. Operators had their fingers and hands exposed to unexpected cutter start-up hazard likely to result in amputation injury when accessing the shear point to pull material through. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
