29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit required confined space entry program that complied with 29 CFR 1910.146: (a) In the Repair and Coating/Painting Departments, on or about December 15, 2017 and at times prior to, the employer did not properly implement their written permit required confined space entry program by not effectively assigning responsibility of warning tag/device placement on tank cars and not providing sufficient means of communication between departments when employees were required to work in the permit required confined spaces, exposing employees to hazardous atmospheres.
