29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. WORKSHOP: The energy control procedures did not include all the energy sources and alternative measures in the machine specific for the employees of the hosting and contract employers during servicing and maintenance of equipment, such as (but not limited to) the Jacketing EJ extrusion lines On/or about November 28, 2017, an extrusion head that was in the process of being moved from a work table onto a transfer cart fell onto an employee's abdomen and leg resulting in broken bone injuries from the incident. The extrusion head, which was one of hazardous energy sources was not appropriately secured to prevent from displacement.
