29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part: On or about November 15, 2017, at the above addressed jobsite, employees were performing construction type work activities, to include permit required confined space entry activities (elevator pit), wherein the employer did not initiate and maintain safety, health, and accident prevention programs, as may be necessary to comply with this part, such as a: written permit space program; consisting of a written permit space survey by a Competent Person, testing for hazardous atmospheres and an employee training/employee safety behavior modification program/policy, and wherein an employee who was working in an elevator pit, was crushed by an elevator car and was subsequently seriously injured and hospitalized, and also resulted in exposing the other employees working at this job site to permit space physical hazards. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
