29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: a) On or about November 2, 2017 and times prior at the number 2 dryer: The employer failed to ensure employees working to clear a jam on #2 dryer were provided lockout/tagout equipment to allow them to properly lockout the equipment which exposed them the hazards associated with inadvertent start-up and/or running equipment.
