29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about 11/1/2017 throughout the facility full time and temporary operators were exposed to struck-by, caught-in, and amputation hazards where the employer failed to enforce the utilization of specific procedures by employees for activities such as but not limited to cleaning and clearing of the Raw Grinding Line.
