29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person: In the alternative Section 5(a)(1) of the Occupational Safety and Health Act of 1970: (a) On or about October 20, an employee was fatally injured after falling approximately nine feet from a wooden pallet elevated by a Toyota 8FGCU25 forklift after being lifted up to access the roof of a 8 foot tall interior building at 302 Gridley Avenue in Amenia, North Dakota.
