29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a. On or about October 4, 2017, in the red meat cutting area, employees were required to change the blades on the band saws. The employer did not ensure that lockout devices were used to isolate all energy sources, thereby exposing employees to potential re-energization of equipment.
