29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe: (a) R & I Construction, Inc. on or about September 27, 2017, at the site located on Ohio 53, quarter mile north of Ohio 103, McCutchenville, OH: The employer did not implement a rigging inspection program. A double leg synthetic web sling utilized to hoist components of a mobile crane was not inspected for deficiencies prior to use. Multiple snags and cuts were present contributing to sling failure under load, exposing employees to struck-by injuries.
