29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by guardrail systems; safety net systems; or personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems. a) Bark Transfer Auger platform: On or about September 27, 2017 and times prior, employees accessed the working platform of the bark transfer auger to clear jams from the front of the chute of the auger system. There was a 61 inch opening between the platform and the structural member of the platform system exposing employees to a fall of 16 feet platform to ground. b) Standing on Auger pipe: On or about September 27, 2017 and times prior, employees stood on the auger pipe to clear jams from the chute of the auger system. There was a 28 inch opening between the top of the pipe and the structural member of the platform system exposing employees to a fall of 18 feet platform to ground.
