29 CFR 1910.261(b)(1): The employer did not ensure that the main power disconnect switch or valve, or both, controlling its source of power or flow of material, was locked out or blocked off with padlock, blank flange, or similar device before conducting any maintenance, inspection, cleaning, adjusting, or servicing of equipment (electrical, mechanical, or other) that required entrance into or close contact with the machinery or equipment: Seaman Paper Company, 51 Main St, Otter River, Massachusetts, Machine 1; On or about September 14, 2017 an employee was permitted to perform troubleshooting on Machine 1 without the machine being locked out, exposing the employee to rotating equipment, in-running nip points, and lacerations from the slitters.
