29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: (a) On or about September 14th, 2017 and at times prior employees were exposed to a struck-by hazard from overhead soil while installing a water line in a trench approximately nine and a half feet deep where two spoil piles were placed directly on the edge of the excavation in the vicinity of 4099 South ND Highway 3 in Dawson, North Dakota.
