29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) SVAS 5 Machine: On or about September 12, 2017 and at times prior thereto the employer exposed his employee to caught-in, struck-by, and amputation hazards in that an employee was allowed to enter a SVAS 5 machine cell while the machine was energized to clear a jam. The company had not developed, documented and utilized procedures for the control of potentially hazardous energy.
