29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. a. Miller Pipeline, LLC jobsite located at 234 Hawley St., Toledo OH: On or about August 16, 2017, the employer did not ensure employees attempting to remove and aiding in the removal of a concrete cap, which was covering a high voltage buried electrical line, were trained on the electrical hazards involved with jackhammering near a buried electrical line, work practices to use, and the protective equipment that must be worn when exposed to electrical hazards.
