29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146: The employer is failing to protect employees from hazards during permit required confined space entry as the employer did not implement a permit space entry program for employees entering sanitary sewer manholes during sewer and drain cleaning. The condition was most recently observed at the intersection of Dorsett and Fee Fee Road, Maryland Heights, MO when an employee entered a sanitary sewer manhole to visually inspect the sewer line after dislodging an obstruction.
