Text For Citation: 01 Item/Group: 001A Hazard:

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about July 21, 2017, and at times prior thereto: a. In the forging hammer area where employees were exposed to struck-by hazards while working near the point of operation of an 8,000 pound forging hammer without a guard to protect employees from the point of operation or flying chips and sparks. b. In the forging hammer area where employees were exposed to struck-by hazards while working near the point of operation of a 16,000 pound forging hammer without a guard to protect employees from the point of operation or flying chips and sparks. In the Alternative OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that wer causing or likely to cause death or serious physical harm to employee in that employee were exposed to being struck-by flying chips of red hot metal. On or about July 21, 2017, and at times prior thereto: a. In the forging hammer area where employees were exposed to struck-by hazards while working near an 8,000 pound forging hammer without protection from flying chips and sparks created during the hammering action. b. In the forging hammer area where employees were exposed to struck-by hazards while working near a 16,000 pound forging hammer without protection from flying chips and sparks created during the hammering action.
