29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Scissor Lift Conveyor - The employer did not develop, document, and utilize energy control (lockout) procedures to be used when servicing the Kopper's conveyor scissor lift. On July 15, 2017, an employee suffered fatal injuries while servicing the Kopper's conveyor scissor lift which was not properly locked out. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement. -------------------------------------------------------------------------------------------------------------- AVD CHANGED DUE TO FSA - 4/9/18 TO THE FOLLOWING Modify AVD to a) Scissor Lift Conveyor The employer did not document energy control (lockout) procedures to be used when servicing the Koppers conveyor lift on or about July 15, 2017.
