29 CFR 1910.23(b)(9): The employer must ensure ladders are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury; On or about June 28, 2017 in the Columbian Bin, an extension ladder was not inspected prior to use to identify defects such as but not limited to worn ladder feet and missing top section of the extension ladder exposing employees to slip and fall hazards.
