29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Perry Tool & Die Co. at 11606 Reed Ct., Broomfield, CO 80020: On and before June 23, 2017 the employer did not ensure that procedures were developed, documented and utilized for the control of energy when employees were required to place any part of his or her body into the point of operation. This condition exposed the employees to crushing and amputation hazards during troubleshooting and maintenance activities. This condition includes but is not limited to: (1) Amada press brake models: RG-50, RG-100, RG-35S, and RG3512LD (2) Amada punch press models: 110, Pega-244, VIPROS 255, and VIPROS 357 (3) Amada sheer model: M-3045 (4) Perkins Machine Company punch press model: 300-B (5) Rother Machine Tools press brake, S/N 10VSG/A2066
