29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part: On or about June 22, 2017, at the jobsite located near 7727 Horatio Drive, Saint Louis, Missouri, the employer did not initiate and maintain training programs that adequately provided guidance to ensure the prevention of exposure to amputation hazard(s). An employee sustained an amputation to the tip of the ring finger of the right hand while operating the IST Reel (Make: Ritec; Model: Motorized Cable Reel XXL; S/N: 81).
