29 CFR 1910.146(d)(3)(vi): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit entry operations, including, but not limited to, verifying that conditions in the permit space are accepable for entry throughout the duration of an authorized entry: On or about September 22, 2017, Tactical Cleaning Acquisition, LLC had employees enter and work within a rail tank car, GLNX 24138, and the employer did not develop and implement procedures and practices to verify that conditions in the permit space were acceptable for entry throughout the duration of authorized entry.
