29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a) Fire Service Safety, Inc. located at 628 Township Road 95 in Huntsville, Ohio; On or about June 2, 2017 the employer did not ensure the employees working on walking-working surfaces with unprotected sides was protected from falling by means of, but not limited to, a guardrail system, safety net system, or personal fall protection systems. An employee performing power washing activities on the roof of an outdoor patio was exposed to a fall hazard of approximately 8.75 feet to the concrete ground.
