29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Site: in the production area, 4 Springfield Street, Three Rivers, MA 01080, on 4/5/2017 the right hand button for the two hand control station for the pneumatic press did not function allowing operation of the press solely by use of the left button. This allowed access to the danger zone during operating cycles. (b) In the production area, within the facility at 4 Springfield Street, Three Rivers, MA 01080, on 5/26/2017 and at times earlier, the point of operation for the Bridgeport, Series I, 2 HP milling machine was not guarded to prevent employee(s) from having any part of their body in the danger zone during operating cycle(s).
