Text For Citation: 01 Item/Group: 001A Hazard: UNAPEQUIP
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) The hazard of running off the roadway while operating Euclid Water Truck, model number MTT8, serial number 80-1560, on an unapproved non-permitted mine access road in that the road width was less than that recommended for surface mine vehicle operations resulting in an accident on May 6, 1993. Feasible and acceptable abatement methods, among others, to correct this hazards would be to design the roadway in accordance with: (1) "Information Circular 8758, Design of Surface Mine Haulage Roads-A Manual," U.S. Dept. of the Interior, Bureau of Mines; (2) "Highway Design Division Operations and Procedures Manual," State Department of Highways and Public
