29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy controls: On or about May 11, 2017, Agri-Fab, Inc. had an employee perform maintenance operations on a Modern Cut-Off Machine in the Machine Department, and the machine specific written lockout procedure did not contain specific requirements for testing the equipment to determine and verify the effectiveness of lockout devices.
