29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; a. Located in the Extrusion Department of the facility: On or about May 12, 2017 and at times prior, employees were performing service and maintenance activities on extrusion Line Z1, such as, but not limited to, unjamming and cleaning the machine. The developed lockout procedure for extruders and palletizers did not include specific procedural steps to isolate and control each hazardous energy source, such as, but not limited to, lockout point locations, lockout devices to operate, and removal of residual and/or stored energy.
