29 CFR 1910.178(l)(2)(i)(A): The employer did not ensure trainees operated a powered industrial truck only under the direct supervision of persons who have the knowledge, training, and experience to train operators and evaluate their competence: a. Site - On or about April 20, 2017, the employer failed to provide direct supervision over an employee who had not completed the forklift training program and also received serious injuries as a result of a forklift incident.
