29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about April 26, 2017 and at times prior to, employees were exposed to a caught-in hazard where the employer failed to ensure that lockout and tagout procedures were developed, documented and utilized to protect employees performing cleaning of machines such as but not limited to Table-Top Muffin/Cake Batter Depositor "T" Series.
