29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) 400 Seaks Run Road, Glen Rock Pennsylvania, 50 gallon reactor - The employer did not ensure that when employees were troubleshooting, testing or repositioning a machine that a device was affixed to each energy source, on or about April 25, 2017. (b) 400 Seaks Run Road, Glen Rock Pennsylvania, 50 gallon reactor - The employer did not ensure that when employees preformed maintenance on a machine that a device was affixed to each energy source, on or about April 27, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
