29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: (a) GE Johnson Construction Company, 1776 Curtis Street, Denver, CO: On and prior to April 20th, 2017, the creating and exposing employer did not ensure that a 4' X 8' piece of plywood placed over a 3.5' X 3.5' floor hole on the roof of the 29 story building was secured to prevent displacement. An employee who was cleaning and organizing this roof area was seriously injured when he inadvertently picked up the plywood floor hole cover and then stepped through floor hole. The employee fell approximately 12 feet to the floor below.
