Ross Casting & Innovation, LLC located in Sidney, Ohio: On or about May 25, 2017, the employer did not ensure employees secured the semi-trailer in Bay 10 from movement by using devices such as but not limited to the dock lock or wheel chocks, prior to using a forklift to enter and exit the trailer.
