Text For Citation: 01 Item/Group: 001A Hazard:
29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a) On or about April 4, 2016, a maintenance employee suffered the amputation of the tip of their middle finger when it became caught between an in-going nip point while removing a belt from the large Boiler #2's fuel pump motor and fuel pump pulleys. The established Large Boiler #2 lock-out/tag-out procedures were not utilized prior to the removal of the fuel pump belt guard.
