29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph: (a) Division 7, Inc. at 38 Gold Piece, Breckenridge, CO 80424: On and before April 12, 2017, the creating, exposing, and correcting employer did not ensure that a 29" wide supported scaffold with a height to base ratio of more than four to one (4:1) was restrained from tipping. The scaffold platform was used at a height of at least 10 feet to install insulation on a ceiling. An employee experienced lower body fractures when the scaffold he was using tipped over, and the employee fell to the floor. This condition exposed employees to fall hazards of at least 10 feet.
