29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: On or about April 7, 2017, an employee operating the 5 foot panel welder was exposed to caught-in and amputation hazards while attempting to place a moving wire back on a straightener roller and the employer did not ensure that the equipment was turned off or shut down before the employee performed the activity on the equipment.
