29 CFR 1910.23(c)(11): Portable ladders used to gain access to an upper landing surface did not have side rails that extend at least 3 feet (0.9 m) above the upper landing surface (see Figure D-1): a. Site- An employee using an aluminum extension ladder to access the awning over the entryway was exposed to a fall hazard where the ladder did not extend at all above the landing surface.
