29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: (a) On the Mark Andy MA two (2) machine, the employer did not ensure that the control of hazardous energy procedure was used during servicing and maintenance activities including, but not limited to, cleaning and set-up operations, exposing employees to pinch points, caught-in-between hazards and amputation injuries. (b) On the Mark Andy MA three (3) machine, the employer did not ensure that the control of hazardous energy procedure was used during servicing and maintenance activities including, but not limited to, cleaning and set-up operations, exposing employees to pinch points, caught-in-between hazards and amputation injuries. (c) On the Mark Andy MA four (4) machine, the employer did not ensure that the control of hazardous energy procedure was used during servicing and maintenance activities including, but not limited to, cleaning and set-up operations, exposing employees to pinch points, caught-in-between hazards and amputation injuries. (d) On or about March 23, 2017, an employee performing cleaning of a roller on the Mark Andy MA five (5) machine was exposed to pinch points, caught-in-between hazards and amputation injuries, in that the employer did not ensure that the control of hazardous energy procedure was used to prevent inadvertent starting of the machine.
