29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) On or about March 13, 2017 and at times prior thereto, employees were operating a Liftall M80D forklift with a worn clutch which had been modified with a welded on piece of square metal tubing used to extend the clutch pedal reach, where no manufacturers written approval was acquired. Note-Abatement certification and documentation is required for this item.
