29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about January 22, 2017, the employer did not establish and utilize an adequate energy control procedure for employees to safely troubleshoot stoppages, clear fibers and unjam terminal strips on Terminal Machine 4-2 in the cutting area. Employees were exposed to amputation hazards while performing these tasks. No abatement certification or documentation is required for this item.
