OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards while using overhead cranes: a) Fabrication shop and storage/loading yard - On or about January 19, 2017, employees used multiple overhead cranes with under-hung monorail trolleys that had deficient/inoperative brakes and safety latches. Among other feasible means of abatement, Both frequent and periodic Inspection and Testing should be conducted in accordance with ASME B30.17 - 2015, Cranes and Monorails (With Underhung Trolley or Bridge) Chapter 17-2.
