29 CFR 1910.333(a)(1): Live parts to which an employee may be exposed were not deenergized before the employee worked on or was near to them: The employer is failing to protect employees from electrical shock, burns and arc flash hazards. This was most recently documented on December 13, 2016 at 4:30am at 580 7th St. Omaha Nebraska. The employer failed to ensure that the employee de-energized the equipment to be worked on or ensure that precautions were taken to protect the employee from potential electric shock or an arc flash incident. An employee was seriously injured when an arc flash occurred when he was working on or around live electrical switch gear.
