29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and projected more than one half the diameter of the shaft: a) Plant - On or about 10/27/16 and at times prior thereto workers operated the MBI (Metal Building Insulation) lamination machine, and the 1.5 in. diameter shaft of the adhesive roller projected 6.5 in. outside the machine frame and did not present a smooth edge. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
