29 CFR 1926.451(f)(1): Scaffold and/or scaffold components were loaded in excess of their maximum intended loads or rated capacities, whichever was less: On or about October 24, 2016, at the above addressed jobsite, the 5/8 eye bolts used to support the swing stage scaffold had their rated capacities exceeded, causing the eye bolts to fail and the scaffold to fall approximately 40 feet to the ground below.
