29 CFR 1926.251(a)(1): Employer did not ensure rigging equipment for material handling was inspected prior to use on each shift and as necessary during its use to ensure that it is safe. Defective rigging equipment was not removed from service: On September 09, 2016, the employer did not ensure defective powered industrial trucks were taken out of service. The employees performing work were exposed to struck-by hazards.
