29 CFR 1910.26(c)(2)(iv): Ladder(s) are not maintained in a good usable condition at all times. Employees are exposed to fall hazards when using ladders to access the beds of the trucks and the employer fails to maintain ladders in good repair. This violation was last observed on November 15, 2016, in the fabrication shop; employees were observed utilizing an aluminum step ladder with missing and bent horizontal supports between the back rails, to access the bed of a knuckle boom truck. To abate this violation, the employer must ensure that all ladders are maintained in good usable condition. Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including a copy of site-specific program detailing the procedures used to ensure compliance and training of employees.
