Text For Citation: 01 Item/Group: 001A Hazard:
29 CFR 1910.178(p)(1): A powered industrial truck was found to be in need of repair, defective, or in any way unsafe, the truck and was not be taken out of service until it had been restored to safe operating condition. On or about 8/22/2016, employees were operating defective powered industrial trucks. The following forklifts were on site and not removed from service: a) Komatsu Forklift #8, Model FG25St-14; the LPG tank indexing/locator pin was missing and the tank was installed incorrectly. b) Komatsu Forklift # 3, Model FG35BCS-7; the LPG tank indexing/locator pin was missing, the LPG tank was installed incorrectly, the operators seat was damaged and missing padding and a pedal was missing the pad or cover. c) Komatsu Forklift # 10, Model FG25ST-14; the LPG tank indexing/locator pin was missing. d) Komatsu Forklift # 14, Model FG25ST-16; the LPG tank indexing/locator pin was missing. e) Toyota Forklift #2, Model 8FGCU25; the LPG tank locking pin/bracket was damaged. f) TCM Forklift #24, Model FCG368L; one of the brake lights was inoperable and the LPG tank indexing/locator pin was missing. g) TCM Forklift #23, Model FCG30HL; the LPG tank indexing/locator pin was missing and a pedal was missing the pad or cover.
