29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 08/01/2016 - Maintenance department, an employee performing electrical work on machinery such as, but not limited to, a Battenfield injection molding, TR-US 02 with 480 VAC , pneumatic pressure, and gravitational sources of energy was doing so without a specific procedure for locking and tagging out the machinery. b) On or about 08/01/2016 - Maintenance department, an employee performing mechanical work on machinery such as, but not limited to, the potting machine with electrical, pneumatic, and gravitational sources of energy was doing so without a specific procedure for locking and tagging out the machinery. c) On or about 08/01/2016 - Maintenance department, an employee performing maintenance work on machinery such as, but not limited to, the drill machinery with electrical and pneumatic sources of energy was doing so without a specific procedure for locking and tagging out the machinery.
