29 CFR 1910.134(a)(2): A respirator was not provided to each employee when necessary to protect the health of such employee: (a) Harmony Road, Gandeeville, WV: On or about July 17, 2016 and times prior the employer did not provide a supplied air respirator to employees working in and around excavations to fix leaks in natural gas lines that remained pressurized and had the potential for an oxygen deficient atmosphere from the displacement of the oxygen in and around the excavation by the leaking gas.
