Text For Citation: 01 Item/Group: 001A Hazard: EXPLOSION
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards: a.Drivers using a Cosmodyne Turbocraft six inch centrifugal pump, to load liquid oxygen into a storage tank, routinely over pressurized the pump inlet to above the manufacturer's recommended rating of 100 psi. This caused the pump seal to fail on January 18, 1996. Drivers were not instructed as to the proper pumping procedure, and there was not an over pressurization device provided on the pump's suction side. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS: 1. Install a pressure regulator on the inlet side of the pump to insure that the pump cannot be pressurized above the manufacturer's recommended rating. 2. Instruct the drivers in the proper pumping procedures. 3. Install a pressure relief device set that will prevent over pressurization of the pump.
